To open a new account, please download the prescription form and mailing label and send us your case(s). Your new account will automatically be created upon receiving your case(s). An invoice will be sent to you at the end of each month.

2. SHIP US YOUR CASE(S) - DROP OFF AT YOUR LOCAL USPS POST OFFICE OR ANY USPS MAILBOX
Make sure your models are packaged well so they will not get damaged during transit
Transit time via USPS First Class may take up to 10 business days, schedule enough time for your patient's next visit
Free shipping applies to shipments containing 3 or more cases per box. Shipments containing less than 3 cases per box will incur a fee of $7.00 per case. *

3. WE WILL FABRICATE YOUR APPLIANCE METICULOUSLY FOR DELIVERY ON OR BEFORE THE 'DATE WANTED'
In-Laboratory processing time is typically 24 hours for a new appliance construction. If an unavoidable delay should occur, you will of course be notified immediately.
Return shipping will be via United States Postal Service (USPS) or United Parcel Service (UPS), depending on expected transit time
Tracking numbers can be provided upon request
Rush delivery is available via UPS Next Day Air®, UPS 2nd Day Air® or UPS 3-Day Select®, please call in advance for additional charges *

4. WE WILL SEND YOU AN INVOICE AT THE END OF EACH MONTH
Customers will receive an invoice at the end of each month with a total balance for all products and services provided for the duration of that month
All payments are due within 15 days of invoice date (accounts 30 days past due will result in pending cases being placed on hold)
Amounts over 15 days will be subject to 1.5% per month service charge*, annual rate of 18%
Payment may be made via check or credit card (Visa Card, Master Card, Discover Card or American Express)
Payment must be made at time of order via credit card for all international orders. Please call for more information and details.
